The agency engineer was clearly instructed to remove the accumulated water completely and proceed with concrete casting only after proper dewatering and inspection clearance. 2-Cover blocks were not placed properly at site. 3-Material test reports and approved drawing (hard copies) were not available at site during inspection. 4-During casting, the slump test was conducted twice; in the first test, the slump value was found to be 170 mm, whereas in the second test, zero slump was observed, which is also unsatisfactory. 5-It is a matter of serious concern that concrete casting of the OHT raft was carried out without rectification of the above observations and without inspection approval. Further, an RFI for column up to GL level was raised on 03 February 2026, which is procedurally incorrect and unacceptable, as the raft-related observations were not closed prior to proceeding with subsequent construction activities and all ongoing and further works are hereby stopped with immediate effect.
